Creating Consent Forms


A Consent Form is an optional addition that can be attached to a survey. The researcher has the ability to create their own terms/conditions that the participant must agree to before being able to complete the survey.

You may create as many Consent Forms as you wish for a Workspace.

Creating Your Form:

  1. Select the Consent Forms icon from the Workspacetray.
  2. Click Create Consent Form and simply copy and paste an existing form you may have, or create a new one.
  3. Consent Forms can also be created from the Study Settings page. Select your study, scroll down to the Consent Form drop-down box and select Create New Consent Form.
  4. You can create more than one Consent Form and edit their content as you feel.

  5. To delete a Consent Form, scroll to the item you wish to get rid of and select the red Delete button.

 

By attaching a Consent Form to your study, participants will not be able to begin answering questions unless they give express consent by agreeing to your outlined terms and conditions within the form.
